I have read and understood UK Farmcare requirements for TB test arrangement as laid out in the Practice operational manual.
I and all staff arranging TB tests have received adequate training to use both TOM (not applicable for Iechyd Da tests) and SAM.

I will ensure that the practice operates a proactive system of booking of tests; that all tests are booked in well within test window or there is evidence of adequate communication attempt to do so.
I will ensure that at least twice a week SAM/TOM is checked for test updates.
I understand the importance of ensuring our OVs and ATTs are supplied with all relevant test information before starting a test.
This includes information about farm handling facilities and H&S issues.
I will ensure that all relevant documents and communications related to each test are filed together and kept for 3 years and 60 days.
